Day Tour #1:  Guided Intracoast Waterway Tour, Saturday, September 12, 2009, $115 per person, includes beverages and snacks. 
Day Tour #2:  Worth Avenue Shopping, Monday, September 14, 2009, $20 per person, includes round trip shuttle service to and from Worth Avenue.
Day Tour #3:  Airboat and Flamingo Garden Tour, Tuesday, September 15, 2009, $200 per person, includes beverages, snacks and a boxed lunch.

Payment is accepted by check, Visa, MasterCard and/or Discover.  Make checks payable to: AMBA Mail your check payment along with the completed form to Centennial Conferences (see address below).  For payment by credit card, please complete the required fields in the payment section of the registration form and send it by mail or fax to Centennial Conferences.

Cancellation Policy
An administrative fee of $80 is retained for any cancelled registration.  No refunds for cancellation after Friday, August 28, 2009.
